homeNO PURCHASE NECESSARY TO ENTER OR TO WIN A PRIZE1) Description of Sweepstakes – Hollywood Tanning Systems Inc. (the “Sponsor”) is offering “Hot Tan Hot Car” (the “Sweepstakes ”) and will award prizes based on a random selection from all valid entrants in the Sweepstakes. The overall promotion begins February 1, 2007 and ends December 31, 2007 (“Overall Promotion Period”). Within the Overall Promotion Period there are eleven (11) separate and independent sweepstakes drawings (“Monthly Sweepstakes Periods”) conducted on the following schedule. Monthly Sweepstakes
2) Eligibility -Sweepstakes is open to legal residents of the United States and the District of Columbia who are 18 years of age or older. Sweepstakes is void where prohibited or restricted by law. Employees and officers and representatives (and members of their immediate families and those living in the same household of each) of Hollywood Tanning Systems Inc., its franchisees, its advertising and promotion agencies and any company involved in the design, execution or production of the Sweepstakes are not eligible. 3) How to Enter -(A) Automatically – You will receive one (1) entry automatically for every Hollywood Tans tanning session you use during a Monthly Sweepstakes Period. Limit one entry per day. (B) By Mail – Request an Official Hot Tan Hot Car Sweepstakes Entry Form at participating locations. Hand print your first and last name, street address (no PO Boxes), city, state, zip code, phone number including area code and email address. Mail the entry form in a #10, (business size) envelope to the address indicated on the form. There is a limit of one Official Entry Form request per person per day; each Official Entry Form must be mailed in a separate hand addressed envelope. Only Official Entry forms are valid for mailed entry; no photo-copies or reproductions of the Official Entry Form are valid. You can also request an Official Hot Tan Hot Car Sweepstakes Entry form by sending a hand addressed, postage affixed envelope to: Hot Tan Hot Car Official Entry Form Request, PO Box 4760, Scottsdale, AZ 85261-4760. Request for Official Entry forms by mail must be received no later than December 31, 2007. All entries, regardless of entry method, are valid only for the Monthly Sweepstakes Period during which the tanning session was used or the mailed entry was received. Entries are not “rolled forward” for inclusion in subsequent Monthly Sweepstakes Periods. 4) Monthly Prizes - One Grand Prize Winner will be randomly selected in each Monthly Sweepstakes Period to receive a 24month, 15,000 annual mile lease of a 2007 Porsche Boxster vehicle equipped with manual transmission and standard features. The Approximate Retail Value (“ARV”) of the 24 lease payments is $21,936. The winner will be required to meet all the terms and conditions of Porsche Financial Services Preferred Lease Agreement, (including mileage limitations). The amount of the twenty-four (24) monthly lease payments and any applicable down payment will be paid by the Sponsor; however the lease will be executed in the name of the winner and the winner assumes all responsibilities associated with the lease. The amount of the monthly lease payments and any applicable down payment does not include title, tag, registration fees or applicable taxes, which are the sole responsibility of the winner. The color of the vehicle is subject to availability of dealer. Actual lease vehicle may vary from promotional images of the vehicle. There are no additional warranties on the vehicle except as may be offered by the manufacturer. Winner must be a licensed and insured driver and will be responsible for picking up the vehicle at an authorized Porsche dealership nearest the winner’s home as specified by the Sponsor. The winner shall be responsible for all other costs and expenses associated with the lease of the vehicle, including but not limited to insurance, fuel, tolls, routine maintenance, excess mileage, excess wear & tear or damage to the car and all applicable taxes. Grand Prize Winner will have the option of receiving the 24-month Porsche Boxster lease prize or a one-time cash payment of $15,000. Twenty-five (25) First Place Winners will be randomly selected in each Monthly Sweepstakes to receive an Apple 1 GB iPod® shuffle, ARV $79. There are a total of twenty-six (26) prizes available in each Monthly Sweepstakes Period with an overall approximate value of all prizes of $23,911. The odds of winning any prize depend upon the number of valid entries received in each of the Monthly Sweepstakes Periods. The total value of all prizes available in all eleven (11) Sweepstakes Periods is $263,021. 5) General -The potential winner of the Grand Prize will be required to complete an Affidavit of Eligibility and a Liability/Publicity/Photo Release (unless prohibited by law). The Affidavit, and Liability/Publicity/Photo Release must be returned within seven (7) days of receipt of these documents. Potential winner will also be required to send a copy of his/her passport, driver’s license, birth certificate or other appropriate documentation (acceptable to Sponsor) as proof of age and residency. Non-compliance with any of these requirements will result in disqualification, in which case an alternate winner will be chosen. Return of prize notification and/or the Affidavit/Release as undeliverable will result in disqualification and an alternate winner will be selected. Potential winners of the iPod shuffle prize will be contacted by phone or mail to confirm prize acceptance and verify shipping address. If a potential iPod shuffle winner does not respond within five (5) days of first communication he/she will be disqualified and an alternate winner selected. Prizes cannot be substituted or transferred by the winner and there are no options other than the cash option for the Grand Prize Porsche lease prize winner. In the event of unavailability of any prize, Sponsor reserves the right to substitute a prize of equal or greater value. Warranty on the iPod shuffle prize is limited to the warranty extended by the manufacturer. Limit, one prize per person per Monthly Sweepstakes Period. If, for reasons beyond the Sponsor’s control (including, but not limited to, tampering or computer virus infection), the Sweepstakes is not capable of running as originally planned, the Sponsor, at its sole discretion, reserves the right to cancel or modify the Sweepstakes, without liability and to the extent feasible, will award prizes among entries received prior to cancellation or modification. Sponsor will not be responsible for lost, late, misdirected, damaged, or postage due mail, or for Internet, computer hardware and software, phone, and other technical errors, malfunctions, and delays which may affect the collection of entries. Entries are void if they contain typographical or other errors. Entries, which are mutilated, incomplete, illegible, inaccurate, forged, irregular in any way, or otherwise not in compliance with these Official Rules, are also void. All entrants agree to comply fully with each provision in these Official Rules. Any person attempting to defraud or in any way tamper with this Sweepstakes and any person who does not comply with these Official Rules, will be ineligible for any prize(s) and may be prosecuted to the full extent of the law. By accepting a prize, you grant to Hollywood Tanning Systems Inc. the right, except where prohibited by law, to use your name, likeness, picture, address (city and state), e-mail address, voice, biographical information, and written or oral statements, for advertising and promotional purposes in promoting or publicizing Hollywood Tanning Systems Inc., its franchisees, and its products or services, without compensation unless required by law. You shall have no right of approval, no claim to compensation, and no claim (including, without limitation, claims based on invasion of privacy, defamation, or right of publicity) arising out of any use, blurring, alteration, or use in composite form of your name, picture, likeness, address (city and state), e mail address, biographical information, or entry. 6) Release of Liability and Issues of Law -By participating in this Sweepstakes, all entrants release Hollywood Tanning Systems Inc., its franchisees, affiliates, partners, subsidiaries, officers, directors, agents, employees and all entities associated with the development and execution of this promotion from any and all liability with respect to and in any way arising from participating in this promotion, acceptance or use of prizes. All Federal, state and local laws and regulations are applicable. THE NEW JERSEY COURTS (STATE AND FEDERAL) SHALL HAVE SOLE JURISDICTION OF ANY CONTROVERSIES REGARDING THE SWEEPSTAKES AND THE LAWS OF THE STATE OF MINNESOTA SHALL GOVERN THE SWEEPSTAKES. EACH ENTRANT WAIVES ANY AND ALL OBJECTIONS TO JURISDICTION AND VENUE IN THOSE COURTS AND HEREBY SUBMIT TO THE JURISDICTION OF THOSE COURTS. The Sweepstakes is void where restricted or prohibited by law. 7) Winners List -For a list of winners, send a stamped, self-addressed envelope to: Hot Tan Hot Car Sweepstakes Winners List, PO Box 4760, Scottsdale, AZ 85261-4760. You will receive the most current accumulative winner’s list for all Monthly Sweepstakes completed based upon the date we receive your request.
